Tammara Lynn Fracker Smith, 41, of Nashport, went home to be with her Lord and Savior at 5:03 p.m. Tuesday, December 4, 2012 at Genesis Good Samaritan Hospital. She was born October 26, 1971, in Zanesville, a daughter of Rex (Brenda) Fracker of Nashport and Susan Lyons (Randy) Wolff of Zanesville. She was employed as a nurse at Newark Health Care.
In addition to her parents, she is survived by her husband, Pat Smith of the home; a son, Kyle Patrick Smith; three daughters, Madison Lynn Smith, Laikyn Jayde Anne Smith and Angeleena Procacci Smith; three brothers, Craig Fracker, Randy Wolff and Jeff Wolff; a sister, Tisha Fracker; numerous uncles, aunts, nieces and nephews; a special friend, Janie Yoakam; and her dog, Gizmo.
She was preceded in death by her maternal grandparents, Leonard and Patricia Lyons; and her paternal grandparents, Clarence and Edna Fracker.
Calling hours will be held from 2-4 & 6-8 p.m. Friday, December 7, 2012 at THE SNOUFFER FUNERAL HOME, 1150 W MILITARY RD., ZANESVILLE. Funeral services will be held at 11:00 a.m. Saturday, December 8, 2012 in the Snouffer Chapel with Rev. Joe DiVincenzo officiating. Tammara will be laid to rest in Zanesville Memorial Park Cemetery.
A memorial fund for her children will be set up and envelopes will be available at the funeral home.
