Fatal crash in Fairfield County

Fatal Crash

Lancaster, OH On July 9, 2026, at approximately 7:33 P.M., the Ohio State Highway Patrol Lancaster Post received a call of a two-vehicle crash on Bremen Road SE near Lake Road SE in Berne Township, Fairfield County.

Through the investigation, it was determined that a 2023 Kia Forte driven by Miranda M. Maughmer, 25, of Lancaster was traveling east on Bremen Road. A 2015 GMC Acadia driven by Faith E. Irwin, 33, of Bremen was traveling west on Bremen Road when the Kia Forte traveled left of center and struck the GMC Acadia.  

Ms. Maughmer was transported by Lancaster EMS to Fairfield Medical Center where she succumbed to her injuries. Ms. Maughmer was not wearing a safety belt. She had two juvenile passengers in the rear of the vehicle. A 3-year-old and 8-month-old were transported by Lancaster EMS toFairfield Medical Center. The 3-year-old succumbed to his injuries at Fairfield Medical Center. The 8-month-old sustained minor injuries. Both children were secured in child safety seats at the time of the crash.  

Ms. Irwin was transported by Berne Township EMS to Fairfield Medical Center with non-life-threatening injuries. She was not wearing a safety belt at the time of the crash.

The Ohio State Highway Patrol was assisted by the Fairfield County Sheriff’s Office, Lancaster and Berne Township Fire Department, Fairfield County Engineers Office, Fairfield County Coroners Office, and Esteps Towing.

The crash remains under investigation.
