Fatal accident on US 40 in Muskingum County

Hopewell Township -The Ohio State Highway Patrol is investigating a three vehicle fatal accident.

The crash happened Wednesday, October 22, at approximately 5:10 PM on US 40 near Chapel Hill Road.

According to the Zanesville Post,  Kathryn Bollinger, 74, of Zanesville was operating a 2015 Chevrolet Malibu westbound on US 40. The vehicle struck a 2015 Honda Accord, operated by Gage Summers, 22, of Coshocton. The Honda Accord was stopped on US 40 attempting to make a left turn into a private drive.

Authorities said Bollinger then traveled left of center striking a 2014 Honda Odyssey that was operated by Samantha Mellott, 38, of Zanesville.

Authorities said Diane McElfresh, 73, of Gratiot was a passenger in the Chevrolet Malibu and was pronounced deceased at the scene.

The crash remains under investigation at this time.

Assisting on scene were Falls Township Fire and EMS, Genesis Community Ambulance, Zanesville Fire and EMS, and Ohio Department of Transportation.
