Southeastern Ohio Health Collaborative Urges Community to Complete Survey for Improvement Plan
ZANESVILLE, OH – The Southeastern Ohio Health Improvement Collaborative (Genesis HealthCare System, Morgan County Health Department, Noble County Health Department, Perry County Health Department, and Zanesville-Muskingum County Health Department) is asking for your input to identify health needs in our area.
The collaborative is asking residents to take a short, 10-minute health assessment survey, which will help shape their next health improvement plan. They’re aiming to collect 400 responses by the end of November.
“Any person (18 years and older) in Muskingum, Noble, Perry, Morgan, Guernsey, and Coshocton County can take it. When you are taking the survey, just make sure you select the zip code that you live and/or work in so that way it’ll go to the appropriate county. It is all confidential,” Community Health Planner Michelle Shroyer said.
The survey results will help local health organizations understand the needs of the residents so they can work with local partners in developing programs to improve public health.
“This is important because it will form the Muskingum County Community Health Improvement Plan. This is going to help guide us for the projects for the next three years the county will be working on,” Shroyer said.
